Service Coordinator

Position Description 

• Responsible for facilitating the provision of collaborative, family-center, community-based services through the coordination and collaboration among providers of services for families with infants/toddlers with developmental disabilities
• Monitors and coordinates services as set forth by the Individualized Family Service Plan
• Ensure parent satisfaction with provision of services
• Conducts home visits in accordance with referral to Early Intervention (EI) program
• Links families to community resources based on familial needs
• Responsible for securing and submitting pertinent documentation related to child’s services
• Develop working relationships with community programs, as well as Early Intervention Program constituents
• Maintain extensive documentation in compliance with New York State regulations

Position Requirements 

• Must be equipped to perform the SC duties as listed above from the individual's home.  Equipment requirements include: full computer workstation, internet access, printer, scanner, fax, phone and secure storage for case files.
• Must have the following Windows operating systems for remote access compatibility: Windows 2007 and/or Mac. 
• Must be highly organized, self-motivated and possess superior time-management skills
• Ability to work independently
• Willingness to travel to multiple boroughs
