Job Title: Licensed Practical Nurse

Job Summary: Provides for clients’ well-being and independence by performing nursing duties; coordinating client care; consulting with team members and caregivers on client care issues.

Reports To: Program Manager

FLSA Status: Non-exempt

ESSENTIAL FUNCTIONS of the job:

  • Addresses client health care needs by performing assessments, treatments and interventions (including footcare); consulting with caregivers, physicians and team on treatment; administering medications. Blood pressure, pulse, weight, temperature, heart and lung sounds, vital signs, diabetes checks, etc.;
  • Maintains continuity of care by documenting and communicating actions and information to meet WAC guidelines and/or Full Life Care policy;
  • Maintains continuity of care by communicating changes in client condition to team members; informing caregivers and physicians; documenting changes and service plan events; monitoring progress toward service goals and adjusting services as required;
  • Identifies client care requirements by establishing rapport with clients; completing assessments; interviewing clients, caregivers, physicians, team members and others;
  • Provides team with health care expertise by participating in team discussions; relaying health care information; consulting with team members on health-related client issues;
  • Maintains safe and clean working environment by complying with policies and procedures, universal precautions and other measures to prevent the spread of illness; adhering to medication administration and storage procedures;
  • Promotes client independence by establishing client goals; teaching clients and caregivers to understand condition, medications and self-care skills; answering questions;
  • Maintains nursing supplies inventory and placing orders as necessary;
  • Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner;
  • Trains new staff by giving basic overview of Full Life Care nursing services; demonstrating proper use of Personal Protective Equipment;

MARGINAL FUNCTIONS of the job:

  • Complete projects and tasks as assigned by supervisor;
  • Actively participates in quarterly nursing meetings;
  • Administers annual PPD screens to staff, volunteers and clients;
  • Recieves consultation from RN staff as directed by Supervisor, the WAC for Adult Day Health, and the RCW for nursing;

Educational Requirements Needed to Perform the Duties of the Job:

1. Must be licensed in Washington state as a Licensed Practical Nurse and have one or more years of experience in a health or social services setting;

2. Must have valid CPR certification and complete the HIV/AIDS information course;

3. Computer experience preferred;

Physical Requirements Needed to Perform the Duties of the Job:

Must be able to lift 25 lbs. and support clients’ mobility
