Job Description:
Under direct supervision of a physician working within a clinic admixture room, performs the technical, nonjudgmental aspects of medication preparation, distribution, record keeping and inventory control. Tasks are subject to sequential or final verification by a registered nurse, mid-level provider, or physician.
JOB RESPONSIBLITIES
Prepares medication orders by compounding or admixing medications under supervision of a physician. Checks all orders for completeness of information. Verifies patient information and proper physician authorization.
Checks all orders for insurance approval before admixing.
Able to perform basic pharmaceutical calculations.
Under a professional's supervision, admixes medications as ordered by physician or mid-level provider in an accurate and timely manner as per department guidelines, and preparing all drugs within OSHA and USP guidelines.
Properly record lot numbers, expiration date and volumes of all medications used in compounding for each product prepared.
Maintains patient drug assistance program, and replacement drug program as assigned, including maintaining proper documentation.
Counts controlled substance medication inventory weekly and maintain records.
Maintains current drug information and MSDS sheets.
Properly maintain the medication preparation area, including USP 797 required cleaning per policy.
Checks drugs monthly for outdate and other quality assurance activities as assigned, and maintains accurate documentation to support these activities.
Keeps accurate temperature logs.
Responsible for complete inventory control of medication and related supplies, including ordering, receiving and shelving of all products.
Responsible for maintaining all records related to medications in accordance with state and federal laws.
Able to perform efficient data entry into systems utilized by the department including but not limited to inventory control system.
JOB QUALIFICATIONS
Experience: Two years within the last five years in a comparable job classification with chemotherapy compounding experience.
Education: High school diploma or equivalent.
Licensure: Current Arizona pharmacy technician license
National pharmacy technician license
