Job description

The Physical Therapist is responsible for providing general physical therapy care to CMC’s target population as part of an integrated care team model.  The role of the Physical Therapist is to work as part of the Health Care Team to help restore patient's function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy. The Physical Therapist reports to the Chief Medical Office and/or Specialty Director.

MINIMUM REQUIREMENTS:

          Licensed as a Physical Therapist in the State of California

          Graduate from an accredited Physical Therapy school.

                      Must have a valid and unrestricted California Practice License.

          Knowledge and skills in delivering Physical Therapy to patients

          Interest in designing and evaluating an efficient and patient centered health care system in a community health center setting highly desirable.

          Authoritative knowledge of the principles of practice and general management, specifically in personnel management and techniques in running a Physical Therapy practice in a community health center are highly desirable

          Preference will be given to individuals who have worked with low-income and minority community members

          Valid California driver's license, proof of auto insurance and personal transportation

          Current Cardiopulmonary resuscitation (CPR) Certificate                                

SPECIFIC DUTIES:

          Promotes and believes in Community Medical Center’s mission.

          Develops care planning by consulting with physicians, NPs, PAs, nurses, social workers, and other health care workers and contributing and participating in patient care conferences.

          Contributes to team effort by accomplishing related results as needed and communicates patients’ treatment plans and progress to the primary care team.

          Provide direct one on one Physical Therapy care for CMC’s patients.

          Conducts group appointments with CMC’s patients for the purpose of education with a goal to improve patient functioning and decrease pain.

          Assist the CMO and/or Specialty Director in establishing departmental policies and quality improvement procedures, and evaluating current practices, policies and procedures.

          Plays an active role in CMC’s Pain Management Program, and works directly with the Pain Management Task Force, Primary Care Providers, and Behavioral Health clinicians to provide appropriate care and further develop and improve corresponding policies, protocols and practices.

·                  Meets the patients’ goals and needs and provides quality care by assessing and interpreting evaluations and test results;

·                  Determine physical therapy treatment plans in consultation with physicians, NPs, and Pas, or by prescription.

·                  Helps patient accomplish treatment plan through use of therapeutic devices and manual exercises; instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, daily-living activities, and use of assistive and supportive devices, such as crutches, canes, and prostheses.

·                  Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications.

·                  Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.

·                  Documents patient care services by charting in patient and department records.

·                  Maintains safe and clean working environment by complying with procedures, rules, and regulations.

·                  Protects patients and employees by adhering to infection-control policies and protocols.

·                  Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

          Recommend changes in all clinical areas, and/or in general areas to improve the patient flow, medical records, billing practices, and appointment practices.

          Assist the CMO and/or Specialty Director in designing, implementing and evaluating educational programs for CMC, its patients, and clinical staff.

          Adheres to the highest standards of health care practice, ethics, and professionalism at all times.

          Other duties as required by the funding agencies to carry out contractual agreements and/or as established by CMC’s Management and Board of Directors.

(a)            Must become familiar with the Federal and State rules, regulations, and practices in a Community Health Center.

(b)            Become familiar with Federal and State Health care program requirements and assist in carrying out these programs.

(c)            Assist in developing the health care plan and review of the program requirements, and recommend changes when necessary to carry out the health plan as required.

(d)            Anticipating future legislation; enforcing adherence to requirements; advising management on needed actions

          Ability to relate to the public regardless of ethnic, religion, gender, sexual orientation, or socioeconomic status.

          Assist the Specialty Director and Billing Department in reviewing and recommending Physical Therapy department fees for services, scope of care, and departmental policies

          Serve as a preceptor to Physical Therapists in training as needed

·                  Maintains patient confidence and protects clinic operations by keeping information confidential.

          Performs other duties as required or assigned

                      Knowledge of radiologic studies and interpretation that meet or exceed the standards of Physical Therapy.

                      Knowledge of organization policies, procedures, stems and objectives

                      Knowledge of Physical Therapy care administration systems

                      Skill in exercising initiative, judgment, problem-solving, decision-making

                      Skill in developing and maintaining effective relationships with medical, dental, behavioral health, administrative staff, patients and public

                      Skill in effective documentation and efficient utilization of electronic Health Record

 

TYPICAL PHYSICAL DEMANDS:

The position requires sitting, standing, some bending, stooping, and stretching.  Requires eye-hand coordination and manual dexterity, and lifting ability sufficient to perform Physical Therapy procedures, and operate other office equipment.  Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts.  Requires working under stress in emergency situations or during irregular hours.

TYPICAL WORKING CONDITIONS:

Frequent exposure to communicable diseases, toxic substanc­es and other conditions common to a Physical Therapy clinic environment.
