Job description

The Long-Term Care Project Manager will be responsible for the recruitment of long-term care communities across areas of Ohio to participate in our Phase 3 research study around the adoption and implementation of Dementia-Care Practice Recommendations (DCPR) best practice. They will recruit, participate in all aspects of the research study, build relationships with long-term care communities, lead half of their caseload through a coaching intervention and track, and maintain relationships with the other half of the caseload not receiving coaching. The Long-Term Care Project Manager, with support from their supervisor, will manage the data tracking, entry and all engagements with the care communities including monthly coaching visits, telephone and email supports, grant reporting and other contacts as needed.

There may be significant amount of travel within the state of OH. There are 3 positions available and projected to end June 2024.

Responsibilities

Essential functions and responsibilities include, but are not limited to:

  • Establish effective communications with professional caregiver teams to provide coaching on DCPR, via monthly in-person and/or video conferencing meetings and telephone and email support.
  • Recruit long-term care communities (total of 80 with a team of 3 coaches total) across the state of Ohio.
  • Establish effective communications with professional caregiver teams in the control group to complete all requirements of the research model.
  • Participate in all relevant training focused on coaching skills and understanding DCPR.
  • Participate in research-driven discussions and put recommended DCPR tools and strategies into place.
  • Represent the Alzheimer’ Association chapter at appropriate and assigned in-person and virtual meetings and events in a professional manner.
  • Performs other duties assigned.

Qualifications:  

  • A Master's Degree in social services or a related field preferred or equivalent experience. Experience working with or for a residential setting is a plus.
  • A minimum of two to three years of experience in community outreach activities, and competency in teaching, training and/or delivering educational programs or public speaking/group facilitation.
  • High level of proficiency needed, ability to work in recording and monitoring systems required.

Knowledge, Skills and Abilities: 

  • Knowledge of coaching, group facilitation, and community engagement required.
  • Knowledge of Google Suites and other data management systems strongly preferred
  • Knowledge of long-term care preferred
  • Excellent written, verbal and interpersonal communication skills.
  • Identify and meet the needs of diverse communities and populations within the context of the skilled nursing home setting.
  • Able to learn, teach others, and effectively utilize technology and databased tracking systems.
  • Ability to lead, guide and inspire nursing home staff to achieve goals and objectives while demonstrating and promoting open communication, transparency and teamwork towards the goals of the grant.
  • Ability to multitask, be flexible, and handle changing priorities in a fast-paced environment.
  • Ability for overnight travel, evenings and weekends as needed.
  • Understands and is able to describe and adhere to cultural change and person-centered care practices.
  • Competent in providing evaluation, assessment and guidance on grant guidelines and methods.
  • Access to a reliable vehicle, a valid driver’s license and proof of automobile insurance
  • Ability for travel and to work evenings and weekends as needed.
