Fedcap Rehabilitation Services, Inc. (Fedcap), a New York City-based not-for-profit human services agency, is contracted by the New York City Human Resources Administration to operate the Wellness, Comprehensive Assessment, Rehabilitation and Employment (WeCARE) Program in Manhattan and Bronx.
WeCARE is an internationally acclaimed workforce development program founded and administered by the New York City Human Resources Administration (HRA). WeCARE provides a continuum of integrated services designed to help public assistance recipients overcome barriers to employment and to become more financially self-sufficient. Program services include medical, mental health and substance abuse assessments and referrals; case management; comprehensive service planning; vocational evaluations; federal disability program application preparation and submission; work readiness and computer skills training; academic and vocational education referrals; work experience internship placement and monitoring; and job placement and retention services.
The program is expected to serve more than 20,000 clients annually at a combination of six medical and program service sites in Manhattan and Bronx.
Position Summary:
The CRT RN will review prior medical and vocational testing records, evaluate newly presented medical documentation, and interview the Fedcap client. The interview will focus on the client's current bio (medical) and psychosocial condition. Based on an evaluation of the client's current and prior biopsychosocial condition, the RN will determine the client's employability status.
Duties and Responsibilities:
Serve as a medical subject matter expert, with whom other Fedcap case managers will consult
Based on consultation with the CRT Physician, refer a client for a new, thorough medical examination and assessment at an affiliated Fedcap medical clinic.
Establish work limitations/ accommodations (including a need for travel limitations/ accommodations) for all employable clients
Set goals to bring the temporary disabled clients back to employability.
Respond to workplace medical emergencies
Other department duties as needed.
Qualifications:
RN degree with an active NYS RN license.
A good General knowledge of adult medical and psychiatric conditions
Excellent listening, communicating and documenting skills
Ability to become fluent in reading and documenting results in AllSector (our medical record keeping program) and NYCWAY (HRA's participant record keeping program)
Highly organized and detail-oriented.
Excellent analytic and decision making skills
Excellent verbal, written and interpersonal communication skills.
Ability to be patient and sensitive to individuals with disabilities.
Must be computer literate with a working knowledge of MS Word.
Bi-lingual (English-Spanish) candidates preferred.
