Position Summary:

  • Provide care management and direct nursing services to residents.
  • Ongoing oversight and direct care to assigned residents in regard to their health and medical needs.
  • Follow a prescribed nursing care plan for individual residents.
  • Evaluation, intervention, and modification of nursing care plan as appropriate.

Position Responsibilities:

  • Daily communication regarding all medical needs and issues of the patients (e.g. medical appointments/changes/cancellations, medication changes, resident complaints).
  • Review/follow up on all developed care plans for consumers who require nursing care and maintain awareness of resident's condition.
  • Schedule medical appointments as required and prepare all documentation necessary to take with resident to the appointment.
  • Complete required documentation regarding medical appointments including information for communication hook, sending prescriptions to pharmacy and documenting the consumer's tile.
  • LPN's obtain all medical reports, lab work, etc. and place them in the RN's folder.
  • Transcribe all monthly MAR's and checking the pre-printed MAR's for accuracy.
  • Complete daily check of medications in the medication cabinet.
  • Track consumers' medication supply to ensure that consumers do not run out of medication, order medications as necessary from the pharmacy.
  • LPN's administer insulin injections for specific residents under the direction of the RN.
  • Direct contact with consumers on a regular basis to observe their health care/hygiene status and reports findings to appropriate director.
  • Monitor residents' medical conditions through ongoing observation and diagnostic tools including taking vital signs.
  • Complete and document monthly blood pressure and weight for each resident.
  • Attend Medical Department Meetings, IRA Clinical Meetings, ESI' Meetings held in the ERA and other meetings as deemed necessary.

Essential skills, experience, licensure, certification:

  • Valid NYS Nursing License required
  • Previous experience working with people with developmental disabilities and or psychiatric disabilities preferred.

Valued but not required skills and experience:

  • 1 Year of RN/LPN experience.
  • Specialty certification/training as appropriate.
  • Read and write Basic English with the basic skills necessary to understand and transfer physician's orders.
