ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provides services in accordance with state skilled nursing standards and nursing practices, and maintains compliance with all CCPH community policies and procedures as well as federal and state regulations.
  • Participates in pre-admission and move-in assessments, reviews, and revision assessments based on changes in resident condition or at intervals determined by the state.
  • Administers and documents medication in accordance to the five rights: right medication, right route, right resident, right time, and right dose.
  • Reviews and transcribes physician orders and provides medical treatment as ordered.
  • Supervises and monitors medications for residents who self-administer.
  • Observes resident and reports any changes in condition to physicians, healthcare proxies, and supervisors.
  • Understands the resident’s Advance Directives and follows their wishes.
  • Assists residents with meeting their nutritional and hydration needs and documents them in the nursing record accurately.
  • Monitors meal times and nourishments, assists with room trays, and helps feed the residents.
  • Maintains clear communication with residents and family members while maintaining confidentiality of the residents protected health information in accordance with HIPAA Privacy regulations.
  • Provides a high level of quality care and service to all residents and their families in a professional and positive manner.
  • Must attend all required trainings and in-services.

    Education

  • License to practice as a nurse in the state of Texas (RN or LVN)
  • Minimum one (1) year of experience as a nurse, preferably in a skilled nursing setting.
  • Current CPR certification.
  • Knowledge, Skills, and Abilities

  • Language Ability
    • Ability to communicate effectively in writing and speaking the primary language of the skilled nursing residents and team members.
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals, and state regulations.
    • Ability to write routine reports and correspondence.
    • Excellent oral and written communication and presentation skills.
  • Mathematical Skills
    • Ability to add, subtract, multiply, and divide in all units of measurement, using whole numbers, common fractions, and decimals.
  • Cognitive Demands
    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
    • Ability to deal with problems involving several concrete variables in standardized situations.
  • Computer Skills
    • Proficient in Microsoft Windows and Office Suite
    • Ability to use software for electronic medical records for the purpose of documenting care and changes in condition for skilled nursing residents.
  • Competencies
    • Understanding of general nursing principles, concepts of aging, and concerns of elderly.
    • Demonstrated ability to work in a confidential environment.
    • Detail oriented, excellent organizational skills, and ability to follow through.
  • Work Environment

  • Works primarily indoors in a climate controlled setting.
  • Possible exposure to unpleasant odors.
  • Possible exposure to chemicals as identified in the SDS manual.
  • Possible exposure to infections, diseases, and blood borne pathogens.
  • Continued exposure to residents who are ill, confused, irritable, and/or irrational.
  • Subject to potentially hostile and emotionally upset family members, personnel, and visitors.
  • May work beyond normal working hours and on weekends and holiday as required.
