Job description

The Colorado Coalition for the Homeless seeks a Registered Nurse to perform both independent and delegated duties in the provision of providing comprehensive health care to all patients who present to Stout Street Health Center. This position is located at 2130 Stout Street Denver, CO 80205. The position reports to the Manager of Nursing Services.

Essential Job Functions

  1. Provides skilled nursing care according to accepted community standards of care to preserve or enhance the patient’s highest level of functioning.
  2. Ability to perform quick and accurate triage assessments as outlined in SSHC nursing protocols.
  3. Ability to enter triage patient data and numbers and retrieve that data on a weekly basis for analysis with Manager of Nursing Services, Medical Director, and Revenue Cycle team as appropriate.
  4. Collaborates with other members of the multi-disciplinary health care teams.
  5. Ability to work day or evening clinic as scheduled or appropriate.
  6. Maintains EHR core competencies as evidenced by:
  • Uses telephone template to record communications regarding a patient.
  • Records all patient visits into Nextgen, helping to ensure that PMH, PSH, FH, Social History, and allergies are recorded.
  • Uses inbox to manage tasks.
  • Records vital signs into Nextgen.
  • Completes intake note and submits a superbill in Nextgen to billing for RN services.
  • Enters immunizations into Nextgen.
  • Ability to enter current patient medications and sig into the medication template.

Qualifications Summary

  1. AD, BSN, or MSN in nursing from an accredited college or university.
  2. Current licensure from the State of Colorado to practice as a Registered Nurse.
  3. Certification in Basic Life Support.
  4. Successful completion of the privileging and credentialing process for RNs at the Coalition.
  5. Preferred experience in ambulatory, public or community nursing.
  6. Preferred experience in emergency room, urgent care, or critical care.

Our History
CCH was established in 1984 as a 501(c)(3) non-profit corporation. It was founded by a group of individuals with a will to take action on behalf of Colorado's homeless. They recognized the struggles of working people living in poverty. And they refused to tolerate the dangers faced by those people sleeping in their cars, in tents, or on the streets.

More than 30 years later, many of the underlying social causes of homelessness persist, like poverty, shortages of affordable housing, and unattainable healthcare. The Coalition's integrated housing, healthcare, and supportive service programs have been highly effective at responding to these conditions over time.
