Job description

  • Assures the completion of assessments for assigned individuals within prescribed timeframes and format.
  • Plans, develops, and implements specific developmental training goals, objectives, and methods in area of physical health including self-administration of medications.
  • Assures, in conjunction with the Management of the residential site, the provision of a comprehensive health care plan for each individual. Communicates and coordinates with health care providers, day program providers, families, guardians and advocates. Accompanies individuals to health appointments as needed.
  • Monitors individuals' medical appointments and reviews all consults to assure follow-up on recommendations has been completed and communicated to direct support staff as needed.
  • Maintains a comprehensive healthcare record keeping system in accordance with established policies and procedures.
  • Assures, in conjunction with the Manager, the administration of medications for individuals in accordance Medication Policies and Procedures. Maintains medical cabinets with all approved medication supplies and equipment.
  • Serves as a positive role model for support staff and individuals served. Interacts with others in a way that assists staff and individuals served in developing behaviors, skills and attitudes necessary for success. Maintains neat appearance, hygiene and grooming. Promotes positive, respectful interactions with support staff and individuals served.
  • Provides clinical supervision to assigned LPN (if applicable).
  • Examines individuals to determine health status on a regular basis and when requested. Provides nursing care and first aid for individuals as indicated.
  • Provides initial training to direct support staff to accomplish nursing procedures. Assures availability and completion of documentation which verifies that direct support staff have been educated about the chronic conditions and related health care needs of consumers.
  • Monitors menus to assure dietary needs are met
  • Documents required visits to assigned sites and the activities completed. Assures for RN coverage when absent from assigned sites. Completes on site visit and review of medical needs and medication issues at least one time per week. Submits copy of documented site visits to Coordinator of Nursing Services as required.
  • Develops an individualized Nursing Care Plan for each consumer. Reviews each plan prior to admission and at least annually to assure all areas of medical needs are included and that the Nursing Care Plan is updated whenever there is a significant change in the consumer's condition. Submits Nursing Care Plan to the Coordinator of Nursing Services for review and approval.
  • In collaboration with Coordinator of Nursing Services, Primary Care Physician and Management staff assigned to each residential site assesses which nursing procedures and tasks direct support staff will be permitted to perform. Assures that direct support staff do not perform nursing procedures outside of the scope of practice of an LPN. Assures and documents that all staff are properly trained to perform these procedures.
  • Completes on site observations and annual clinical performance evaluations of all direct support staff’s implementation of nursing tasks and administration of medications. Documents observations and reviews of the direct support performance and forwards a copy to the Coordinator of Nursing and management staff of that site.
  • In collaboration with the management of that site, provide input into the performance appraisal regarding the person's ability/skills to administer medication and perform nursing tasks. Provides necessary training to direct support staff.
  • Implements, maintains, and monitors regulatory compliance in the areas of health, safety and infection control.
  • Provides administrative on call coverage as requested.

EDUCATION and/or EXPERIENCE: Licensed and currently registered by the New York State Department of Education and one year related experience. Applicants are subject to background checks including but not limited to criminal history (fingerprinted), State Central Registry/Office of Children and Family Services, DMV, SEL, MHL 16.35 and various excluded lists including Office of Professionals and OMIG. Must have a reliable vehicle and a valid NYS Driver's license. May be required to meet LAI's Vehicle Operator requirements.


COMMUNICATION SKILLS: Ability to read, analyze, and interpret policies, procedures, clinical plans, individual service plans, and regulations. Ability to write reports, business correspondence, procedure manuals, and other required communication. Ability to effectively present information and respond to questions from groups of consumers, their families, employees, other service providers, the general public and representatives of oversight agencies. Must be able to effectively communicate with consumers, staff and other individuals involved in the support and care of those we serve.

REASONING ABILITY: Ability to solve practical problems and deal with a variety of options in situations where only limited instructions exist. Ability to interpret a variety of instructions furnished in written, or oral form.

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to effectively communicate with and observe consumers, staff, and other individuals involved in the support and care of those we serve.

May be required to meet OMRDD Level II S.C.I.P.-R. (Strategies for Crisis Intervention and Prevention-Revised) requirements.

The employee must frequently lift, transfer, and/or move up to 35 pounds.
