Registered Nurse - Transition of Care

Managed-care company is looking for a Transition of Care RN for inpatient utilization management. This is a unique position in that it is partial work from home, partial inpatient rounds in hospital. Ideal candidate needs to have good hospital navigation abilities, and then ability to review charts and Utilization Management from home.

Responsibilities:

• Monitor inpatient utilization, assisting with facilitation of the discharge or transfer plans and providing follow up to assure that medical care is appropriate and efficient.
• Provide support to the interdisciplinary team to ensure effective resource utilization, as well as quality and cost-effective outcomes.
• Performing telephonic post hospital follow-up to assure members of full understanding of the discharge instructions and current treatment plan.

Required Experience:

• Minimum three years clinical nursing experience with two years managed-care experience preferred.
• Ability to effectively communicate all plan coordination between members, providers, internal departments, community resources, peers, etc.
• Active, unrestricted California R.N. license.
• Valid California Driver's License.

This is a full-time, direct-hire, salaried position with benefits package, Mon - Fri, 8a.m. - 5 p.m.
