Job description

Disability Services of the Southwest is seeking a “Registered Nurse—PRN” to help support our growing San Antonio, TX branch office. We work hard to make sure our clients are receiving the highest quality of care and we strive to be the bridge that empowers their independence.

We are seeking an RN to work with us on a PRN basis, performing assessments and evaluations for our San Antonio area clients in their home environment. Travel reimbursements are provided. As we continue to grow our client base in this region, the potential for growth with this position is also possible!

Essential tasks:

- Supervising direct patient care in compliance with Nurse Practice Act, physicians' plans of care/verbal orders and nursing care plans.

- Managing individual patient's multi-disciplinary needs.

- Coordinating patient multi-disciplinary care needs, re-evaluating the patients’ needs, initiating the plan of care and intervening or revising when appropriate.

- Communicates changes in patients' conditions to appropriate staff/Program Directors, and/or weekend staff, orally or in writing, as needed.

- Prepares comprehensive certification (initial assessments), and re-certifications in the specified time frame as per agency policy.

 

Requirements:

-  Licensed as a Registered Nurse in the State of Texas.

-  Minimum of one year experience as a registered nurse in a home health care setting.

-  Must have a reliable vehicle, auto liability insurance, and the ability to travel around San Antonio, TX.

Founded in 1993, Disability Services of the Southwest (DSSW) is one of the largest providers of support services to people with disabilities and the elderly in the State of Texas. Our services include personal attendant services, nursing and therapies, as well as assistance in home modifications, adaptive aids, job coaching and supported employment.

DSSW is licensed by the state to provide a wide range of services to persons with disabilities including Community Living Assistance and Support Services (CLASS), Consumer Directed Services (CDS), Deaf-Blind with Multiple Disabilties (DBMD), as well as private pay care for the elderly and people with disabilities.

DSSW's programs are designed to assist persons with disabilities in achieving the greatest degree of independence possible within their community. The evaluation process by a nurse and the interview process by a program director and your independent case manager assure that an individual's needs are identified so that they can be met by the services available in each program.

All programs are driven by consumer choice and a people first philosophy.
