The full/time Hospice RN for Admissions performs pre-admission evaluations for appropriateness for hospice services. Initiates the admission process by educating and informing patient/family of hospice and its services.  Also obtains informed consents.   Provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life.  The Hospice RN for Admissions will also be a member of the Hospice Care Consultant Team.

Responsibilities

Receives assignments from the Team Director or Referral Department for evaluation of appropriateness, sign up and or assessments for care.

Formulates the Interdisciplinary plan of care and communicate to the Team Director.

Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

Instructs the patient/family on procedures for contacting hospice.

Makes initial contact with attending physicians, other hospice team members and other agencies as needed to commence care and use of resources for the patient/family.

Maintains daily communication with the Team Directors or Customer Care Department.

Turns in all paperwork pertaining to sign ups and assessments within 24 hours to Clinical Services.

Maintains confidentiality of sensitive information.

Works in conjunction with the Director of Business Development and attends Hospice Care Consultant meetings, working to develop and strengthen provider relationships as part of that team.

Participates in hospice orientation and ongoing education programs.

Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.

Participates on committees and special projects and other related duties as assigned.

Qualifications

Registered Nurse currently and appropriately licensed in the program’s given state(s).

Previous experience in hospice, home health, geriatrics or general medical surgical nursing.

Additional compensation provided for Hospice Certification.

Demonstrated commitment to Seasons Hospice’s philosophy of care, values, mission statement and organizational culture.

Willingness to work a flex schedule and serve patients/families in the geographic service area.

Detail oriented with good time management skills.

Above average documentation skills.

Management/leadership skills.
