Wesley Homes Lea Hill, a non-profit, continuing care retirement community in Auburn, Washington, has a unique opportunity for full time RNs in a newly constructed, state of the art 36 bed Skilled Nursing Center built to provide Transitional, Sub Acute and Long Term Care. The resident centered architectural design provides 34 private rooms and 1 semi private room, configured in a neighborhood concept. It is located near a major medical center yet on a park-like campus, the setting is beautiful. Low nurse-to-patient ratio. Double shift/weekends only position available if desired.
The RN Team Leader promotes and restores residents’ health collaborating with physicians and the interdisciplinary teams; providing physical and psychological support to residents, friends, and families; supervising and evaluating assigned team members such as the CNA. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against resident care goals and standards; making or recommending necessary adjustments; following facility and nursing philosophies and standards of care set by the state board of nursing, state nurse practice act, and other governing agency regulations.

ESSENTIAL FUNCTIONS INCLUDE:
Makes rounds to observe and evaluate the physical and emotional status of each resident at the beginning of each shift and throughout the shift as needed.Alerts the RCM to resident needs that require immediate attention.
Assures that the Certified Nursing Assistant completes resident care activities in a timely, efficient, and appropriate manner that meets the residents’ needs and preferences.
Assists in problem solving of resident care needs with the resident and certified Nursing Assistant. Communicates recommendations and concerns regarding care to the RCM.
Completes performance feedback, evaluations, and corrective action with appropriate staff as assigned and as needed.
Administers medications and treatments in a timely manner and documents as appropriate.Monitors response to medications and treatments and communicates to the physician when necessary.Orders and re-orders medications and supplies when necessary.
Performs the narcotic count each shift with the on-coming shift.Any discrepancies are immediately communicated to the RCM and Director of Clinical services.
Responds to, evaluates, treats, investigates and documents resident accidents/incidents as per facility guidelines. Completes the incident report and submits to the RCM for review.
Documents the residents’ assessments, treatments, skin care, infections and change in condition as per facility guidelines, in the electronic health records system. Places the resident on the 24 hour report and initiates alert charting as per facility guidelines.Contacts the physician, family and/or DPOA when a condition change occurs.
Assures that the CNA’s concerns regarding the resident are listened to, investigated, assessed and treated in a timely manner. Includes the RCM as necessary.Reports the need for plan of care revisions to the RCM.
Assists the RCM in the admission and discharge residents and may participate in resident care conferences.
HIRING PREFERENCES AND REQUIREMENTS:
Registered Nursing (RN) license for Washington State.
High School Diploma or equivalent
2-year college degree in Nursing or related field
Knowledge of Federal and State guidelines along with the WAC’s for skilled nursing facilities.
Previous Nursing experience in a Skilled Nursing Facility.
Experience with IV’s, maintaining pain infusion pumps, and feeding tubes.
Current CPR card.
Able to provide 2 positive professional references.
Able to pass drug screen, DSHS & WSP background inquiries.
